RN Cancer Navigator East Jefferson General Hospital

 The Cancer Nurse Navigator, in collaboration with the Multidisciplinary cancer team, strives to assure cohesive and coordinated patient services. The nurse develops, maintains, and coordinates the multidisciplinary care plan for each assigned patient by incorporating education, community resources and promoting the East Jefferson general Regional Cancer Center. Collaborates with leadership and assigned physicians to continually monitor the level of professional practice related to cancer care and initiates actions or programs to assure comprehensive patient care, team work and professionalism.
