Experience:
Prefer at least one-year experience in a Hospice or other health care setting dealing with seriously and terminally ill persons and family members.Experience working as member of an Interdisciplinary Team.
Two (2) years of Social Work Experience is required.
Hospital experience is preferred.
Education:
Master’s Degree required.
Graduate of School of Social Work approved by the Counsel of Social Work Education.
Licensure/Certification/Registration:
Must hold a current license as a social worker issued by the Louisiana State Board of Social Work Examiners (LSBSWE). This would include either:
·CSW (Certified Social Worker) eligible to take the ASWB master’s exam
·LMSW (Licensed Masters Social Worker)
·LCSW (Licensed Clinical Social Worker)

Responsibilities:
1.Assists in the referral process by obtaining intake information and providing hospice information to patients and/or family.Consults with Hospice Manager to determine appropriateness of admit.
2.Responsible for providing counseling and supportive services to patient/family according to the interdisciplinary care plan and physician's orders.
3.Assesses patient/family coping skills and psychological response to terminal illness and crisis situations.
4.Assists patient/family and staff in appropriate utilization of community resources.
5.Functions as member of the Interdisciplinary Team participating in development and revision of individual plan of care for each patient/family.Meets with Interdisciplinary Team on regular basis.Assists team members in understanding significant social and emotional factors related to management of symptoms and providing psychosocial care to the patient/family.
6.Updates and revises the plan of care as required and provides necessary documentation of services provided.Documents clinical progress notes to be incorporated into the hospice medical record.
7.Assists with bereavement activities as requested by the Hospice Manager.
8.Organizes and conducts family support groups as indicated.
9.Participates in orientation, training, and in-service activities and presents educational programs for staff, volunteers and community agencies; as requested.
10.Assists in planning and implementing an ongoing program for recruitment of volunteers to meet needs of the hospice.
11.Assists in screening, interviewing and checking references on all persons applying for hospice volunteer positions.
12.Assigns volunteers according to patient needs and volunteer skills.Assigns indirect care volunteers to perform non-patient care duties as needed.
13. Consults with other interdisciplinary team members as necessary to determine needs and makes appropriate volunteer assignments.Instructs volunteer on duties according to the Hospice plan of care for each patient/family and provides updates as necessary.
14.Keeps volunteers informed of Hospice events.
15.Participates in continuing education and professional organization and acts as liaison for Hospice with social service and community agencies.
16.Participates in QA activities and serves on committees as needed.
17.Displays competency in applying age-specific criteria in all interpatient relationships.
18.Follows North Oaks Health System’s compliance programs and federal and state regulatory guidelines.Submits only those charges known to be in compliance with the above-stated regulations.
