The Ehlers-Danlos Clinic sees complex patients with symptoms of Ehlers-Danlos Syndrome (EDS). The Clinical Patient Navigator utilizes the nursing process, the healthcare environment and current technology to provide a climate which optimizes patient care and staff development. This individual will work with a team, including Physicians and a Nurse Practitioner, to complete an extensive medical record review and an intake assessment to determine if prospective patients are appropriate for care in the EDS Clinic setting. She/he performs a variety of direct patient care activities appropriate level of training and established procedures. Documents interactions in the patient's electronic medical record. Assist physicians with procedures as requested. Upon completion of visits, this person will ensure that appropriate referrals and follow-up visits and procedures are ordered and completed. The Clinical Patient Navigator will serve as a primary point of contact for patients with questions regarding their care.
