The Community Health Worker functions as a member of a multidisciplinary team that conducts participant/family support to help participants achieve good health practices around cardiovascular disease. Primary responsibilities include facilitation of health improvement, health promotion, health program development and support, disease prevention activities, and coordination with other partners within the community.
WORKING ENVIRONMENT:
This position generally functions within local churches in the Greater New Orleans area with frequent time spent on the computer, or interacting with participants, families, and other health care team members via telephone, electronically, or face-to-face. This position also involves working in the community, including travel within the greater New Orleans Area. Position may require frequent travel between primary locations (assigned Church and Tulane University) and other study partners.
