Catholic Charities- Greenwalt ADHC Registered Nurse Adult Day Health Care/Community Home- The Registered Nurse (RN) position works with Greenwalt Adult Day Health Care Center and Padua Community Homes to include Ocean Community Home, St. Rosalie Group Home, and Sts.
Mary & Elizabeth Group Home. The individual is responsible for staff training for both Greenwalt Adult Day Health Care and Community Home staff.
The RN is responsible for providing oversight to the LPN staff.
The RN reports to the Administrator. Qualifications The Registered Nurse shall have a current Louisiana state nursing license.
Three to five years working with individuals with developmental and intellectual disabilities and disabled seniors.
Knowledge, Skills and Abilities This position required the individual to work in conjunction with staff in the community setting.
It requires the ability to work with and effectively communicate with all levels of staff, care givers and legal guardians.
Understanding of Intermediate Care Facilities and Adult Day Healthcare Centers.
The position requires organizational skills and the ability to coordinate and implement multiple activities at a time.
Must be able to regularly travel to program locations and must Essential Functions Serve on the Interdisciplinary Team and monitor the overall health needs of the participants/clients.
Serve as the liaison between the participants/clients and medical resources, including the treating physician.
Participate on the admission committee to determine appropriateness for placement with CCANO.
Determine the appropriateness of medication regime through observation and evaluation of medication administration.
Monitor and recommend laboratories studies monthly or as needed discuss lab reports with treating physician.
Make referrals to primary care physician for monitoring of specific concerns.
Obtain Instructor certification to teach Certified Medication Attendant and teach the courses as needed.
Monitor the method of administration to participants/clients, both self-administration and staff administration).
Document the receipt of all prescribed medications for each participant/client with a legible signature and comply with all Louisiana laws and rules regarding medication control and disbursement.
Coordinate staff in-service training in accordance to Louisiana Department of Health, Office of Aging and Health Standards.
Coordinate participant/client health related matters at least quarterly.
Monitor and sign off on client Medication Administration Records (MARs) and Physician Orders.
Work directly with pharmacy to make any and all updates and corrections related to clients.
Complete Annual Health Care Plans on participants/clients.
Monitor client/participant charts on a monthly basis Develop and revise nursing policies and procedures as needed.
Train all Direct Service Workers initial 16-hour training.
Other tasks, assignments, responsibilities as delegated by the Supervisor.
